Charges – Apex Gymnastics

Liability of Release
I hereby release all liability from the staff, coaches, contractors , players, and any facility that holds classes, sports, or any other activity in case of injury due to participation in any Apex Gymnastics or Aurum Academy program. I give permission to the staff and coaches to administer any necessary medical attention to my son/daughter in case of injury. I will be notified immediately at the numbers registered with this site if such an occasion occurs. If unable to reach me I give permission to do what is in the best interest of my son/daughter until I am notified. I also understand that my insurance company or I will accept all medical expenses.

Covid Liability Release
I hereby release all liability from the staff, coaches, contractors , players, and any facility that holds classes, sports, or any other activity in the case that myself, my family members, or anyone present with me or with a relationship to me on such a location are exposed to or contract Covid-19 during any Apex Gymnastics or Aurum Academy program or event. I also understand that my insurance company or I will accept all medical expenses related to such an occurrence.

Media Release
I hereby give permission to the staff and faculty of Apex Gymnastics or Aurum Academy to take photos/videos of myself and/or my child and publish them for marketing purposes on Apex Gymnastics or Aurum Academy publications and/or its website. If I wish to opt-out I will contact the office at alex@aurum.ac.
